I, as a parent/guardian consent for my child to take part in the above stated activities and permission will be sought for other activities, including outings. I understand that a register will be taken to acknowledge attendance. I consent to any first aid or emergency treatment required for my child if necessary. I understand that all precautionary measures have been taken to safeguard my child but acknowledge that accidents can still happen. Therefore, I will not hold the organisers responsible for any accident/injury that occurs while my child is in their care. This includes loss and damage to my child’s property.
